Before your first appointment, please print and complete the following Intake and Child's Developmental History Forms. Please bring this paperwork with you to your appointment.
Please also bring a copy of your Insurance Card. As a courtesy to you, we will bill your insurance, however payment is expected at the time of your appointment. We accept Blue Cross & Blue Shield, CHIP Children's Program, Medicaid, Medicare, and Out of Network.
